PAYMENT METHODS
INSURANCE

I currently accept cash, personal checks and Visa/Mastercard/ American Express/Discover cards.

Checks should be made to “Marta Jackson, PLLC.”

You are offered the opportunity to receive a billing statement reflecting charges and payments upon your request.

I am an in-network provider with:

  • Cigna

  • United Healthcare

  • BCBS

  • Aetna

For other insurance panels, I am an out-of-network provider. I can provide you with a receipt to be submitted to your insurance company and reimbursed directly to you. Please, contact your insurance provider for out-of network benefits.

If you are planning to pay for services using insurance benefits, we will further discuss your fees up front. All co-payments, coinsurance, deductibles, and any outstanding balances are due at the time of service.
